Objective: To provide case management services to participants who are eligible for assistance based on level of care guidelines.
Job Description: Case Management consists of services that help consumers gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services.
Case Management includes:
(1) referral and related activities to help the participant obtain needed services;

Qualifications: The Peer Support Specialist is an individual who is grounded in their own mental health recovery. They work one on one to support other individuals on their journey through the recovery process.
